Atypical Carcinoid Tumor with Ectopic ACTH Production used as a Biochemical Marker of Recurrence
نویسندگان
چکیده
Bronchopulmonary carcinoids, including typical and atypical histologies, are uncommon neuroendocrine tumors that have a wide range of clinical phenotypes. In addition to the carcinoid syndrome, other paraneoplastic syndromes are welldescribed, including ectopic adrenocorticotropic hormone (ACTH) production. The authors describe a case presentation of a 23 year-old man with ectopic ACTH production from a bronchopulmonary atypical carcinoid. The use of ACTH as biochemical marker is described here, as its use predicted disease recurrence and helped to guide management.

Cushing’s syndrome associated with typical, peripheral pulmonary carcinoid tumor and N2 lymph node metastasis: case report
Background: The bronchopulmonary carcinoid tumor accounts for 1-2% of all adult malignancies of the lung and 20-30% of all carcinoid tumors. Cushing’s syndrome is the result of chronic exposure to increased concentration of exogenous or endo-genus cortisol hormone, and it is generally associated with central obesity, metabolic syndrome, and hypertension. Treatment is based on decreasing cortiso...
متن کاملEctopic ACTH syndrome due to thymic atypical carcinoid treated with combination chemotherapy of cisplatin and etoposide.
A 21-year-old woman with Cushing's syndrome presented with a mediastinal tumor. Ectopic ACTH syndrome was diagnosed and the tumor was resected. The histopathological diagnosis was thymic atypical carcinoid. Despite all tumor lesions having been resected, the tumor relapsed and multiple metastatic lesions grew aggressively. Combination chemotherapy with cisplatin and etoposide (VP16) was adminis...
متن کاملPulmonary arterial sampling was useful for localizing ectopic ACTH production in a patient with bronchial carcinoid causing Cushing syndrome.
OBJECTIVE We report a 44-year old man with ectopic adrenocorticotropic hormone (ACTH) syndrome caused by bronchial carcinoid that developed Cushing syndrome. METHODS We performed several imaging studies, including chest and abdominal CT, for exploration of nodules and selective pulmonary arterial sampling for localizing a source of ectopic ACTH production. RESULTS The patient was diagnosed ...
متن کاملThymic Atypical Carcinoid with Cyclical Cushing’s Syndrome in A 7-Year-Old Boy: a Case Report and Review of the Literature
Thymic carcinoid tumors very rarely occur during childhood and are often associated with the production of adrenocorticotropic hormone (ACTH), which leads to Cushing’s syndrome. We describe a 7-year-old male patient with a thymic atypical carcinoid tumor who presented with cyclical Cushing’s syndrome. His tumor was initially overlooked on computed tomography scanning in the anterior mediastinum...
متن کاملIPSS in Cyclical Cushing’s Syndrome Due to Ectopic ACTH Production: Proof of Principle and Potential Diagnostic Pitfall
Cyclical Cushing’s syndrome (CCS) has been described in patients with Cushing’s syndrome in which excessive endogenous cortisol is produced in fluctuating rhythmical patterns [1]. This syndrome can be associated with fluctuating signs and symptoms; cycle lengths have been described between 12 hours and 85 days [2]. CCS was initially believed to be very rare, but recent evidence has shown that i...
